What type of input for Fuzzy Logic Controller in Simulink?

Hi,

I want to analyse data using Fuzzy Logic Controller and rule-viewer. My data is in excel which has 2 columns for 2 input parameters for fuzzy logic. Once I had set up my fuzzy logic in FIS editor already. How can I implement this fuzzy logic with my data from excel? And what type of input/output blocks in the Simulink so I can link it with my excel data?
