How do I create a GUI for Simulink Model?

I want to create a GUI for SIMULINK model. I want to enter inputs in a GUI which can be passed on to SIMULINK model. I am not able to link GUI with SIMULINK correctly. Can anyone help me on this?

Does the GUI pass on inputs for the entire simulation at the same time, or does it continuously feed data throughout simulation?
