Blood Vessel Network Extraction

Hi Guys, I have retinal blood vessel where I would like to find a program where I can extract only the blood vessel and exclude all the junk in the background. I tried some of the thresholding algorithms in the "file exchange" but the quality of the output images are not very desirable. can anyone help?
