Segment the whole heart from CT

I am searching for source code to segment the whole heart from CT images.

In CT images, as you can see in the attached image, the regions we want to identify are highly homogeneous in terms of intensity. For example the LV&LA and RV&RA&MYO areas are quite similar in intensity values. Probably what I need is a pattern recognition algorithm that identifies the areas based on their morphological shape rather than intensity.

Please let me know if you find any Matlab code regarding this issue.

It could range from almost trivial (if you can use global thresholding) to extremely difficult. You might want to post your image in a new question.
