How can i draw 3D polar plot for measured data?? plz help me

i measured these data for a transmitting antenna, angle, distance from the antenna and power density of that point,

170 degrees, 5 m , 620 pW/cm2
200 degrees, 50 m, 915 pW/cm2
...........................

like above, how can I draw 3d polar plot for these data. can any one help me?? please
