When using python package, the end user must install 7 gigabytes of MATLAB Runtime?
Or just generate a Python package must install it？

I don't know why you keep mentioning the file size.
Since (to my limited knowledge) there is no way to convert Matlab code to Python, you will need a headless version of Matlab to run the code you wrote. There is no way around that unless you actually do convert the Matlab code to Python.
