Are you kidding? The first 9 lines of this question you describe something, that is solved already. This wastes your and our time.

Then you gave this information: "y=7,6.5,6,5.5,........1,0.5,0.1" and you want "Cause & Effect Diagram" and "Check Sheets". Do you think it is possible to create an answer based on this information? At least I do not think so.
