how can use hysteresis 4 level in simulink

hi. i need some information on how to use comparateur hysteresis , is there any block or any thing related in matlab simulink?

I have 2 input (Error,Hysteresis) for write this function . so how can applic this in simulinc?

because i not find bloc hystersis 4 leve
