Overflows when I use Fixed-point Tool

The Fixed-point Tool indicates an overflow, but I can't find where it is. I set up a scope at the module where the error occurred, but I can't find the overflow point either.

When I extended the simulation runtime, something unexpected happened, and the overflow did indeed exist.
