Lung parenchyma, airway and background have low CT numbers,so a mean value of two peaks (lung parenchyma, airway and fat) is used to binarize the images. After image binarization, 2D hole filling algorithm is used and the thorax is extracted by searching the largest connected components.

Do you have a question? I've given code to extract lungs before - search for it. In the meantime, I attach my demo for extracting the N largest or smallest binary blobs in an image.
