differences between 2D and 3D medical images

i have 2 questions: 1)what is the differences between 2D and 3D medical images? 2)for example assume i have 2D image that contain 1000 patches,each patch is a matrix and i can convert it to a vector,in 3D image how will be patches and how i can convert each patch to a vector?
