How to enter more than one measured disturbances (md) in mpc block for a plant designed entirely in simulink?

Hi,
I have encountered this problem where the plant model is designed in simulink. It has two measured disturbance parameters. But I can't find a way to enter both of them in mpc block.I have tried bus and mux, but this is not working..
