From where should I take output from Phase locked loop

I have a confusion regarding PLL. I understand the concept of locking the frequency of the VCO to the frequency of the coming signal. But from where should the output be taken in case of synchronous Am demodulation and also in FM demodulation? I mean the output is from the filter or from the VCO? Is that output further passed to another circuit to complete the process of demodulation?
