Your function is slightly ambiguous with the usage of brackets, but I get what you are trying to achieve. You need to use the Control System Toolbox from where you can access the tf() and lti/exp() functions to create your desired tf object, then use the LTI block to import that into Simulink. http://www.mathworks.com.au/help/toolbox/control/ref/tf.html http://www.mathworks.com.au/help/toolbox/control/ref/ltiexp.html
Alternatively you could use the System Identification Toolbox if you don't have the Control System Toolbox.

You can use LTI System block from Control System Toolbox as suggested. Another option is to use Transfer Function block in Simulink in series with Transport Delay block. Use the TRansfer Function block for everything, except the delay, and use the Transport Delay block to model the time delay.

What if i need to start the signal with an initial condition (at time = 0seconds, signal amplitude = 2)?

specify that directly from the parameters of the transport delay.
