how to create a unsymmetric profile in a image using pixel values

Hi, iam new to matlab. I've found myself in a problem. I've an image. I want to create a profile in that image through certain region.After making the profile i have to measure the maximum horizontal distane between two points(& also calculated the co-ordinate of that two point). I don't know where to start. Plz help me.
