Where can I find these blocks and help on sine function too

Hello!
How can I create a system like that? Which blocks may I use and how to do the variation below (15% R at 1 Hz)? Is this sine function? I didn't find the 'R1' to work on. I'm working on R2018b.

To me that looks like a Constant block, a Sine Wave block, an Add block, and some other block. That last one the symbols are too blurry for me to identify. If you know the purpose of that block, you could search the appropriate section of the Blocks documentation based on that general description.
Showing a clearer image of that block and/or showing a bit more of the context of how it's used in the model might help others identify what block it is.
